Get Enrolled

To enrol with the practice, you can do one of the following:

• Download the enrolment forms here. Please return the signed form to reception@healthfirst.kiwi or post to:

• Collect an enrolment form from our reception area

Completion of this form does NOT confirm enrolment with the practice. Upon successful enrolment, the practice will book a double appointment with the Doctor for you to go over your medical history (may take up to 2 weeks for your medical notes to arrive). This will incur a charge of $57.00
